So you’ve met with your medical marijuana doctor and you’re ready to get your Kentucky medical marijuana card? Great — you’re already 90% there.
Now let’s walk through the official state registration process to finish it off.

Create Your State Account
Go to the official portal: https://ky-chfs-public.nls.egov.com/register
Fill out your info and submit
You’ll get a confirmation email — check your spam/junk folder if you don’t see it. Click the verification link to activate your account
In the top left corner (under your name), click + Create New Application
Select: “I am a patient”
Click New Patient Registration
Hit Create Application
Under “General Information,” select: “In-state qualifying patient”
Fill in all personal details
Click Save and Next
Enter your physical address → click Verify
Enter your mailing address (if different) → click Verify
Save and continue

Attach Your Doctor Certification
Click “View Available Certifications”
The doctor’s form we submitted will automatically appear in your account
Click Save and Next
If you don’t have a caregiver, just click No
Save and proceed
If you do have a caregiver, they’ll need to register separately and use a different notarized signature page (found in the FAQ section of the state site).

Questions + Legal Attestation
Answer “Yes” or “No” about participating in clinical marijuana studies
Confirm the required “Yes” answers
Sign your full legal name and enter today’s date
Pay the $25 state fee by debit card or bank account (ACH)
Enter your billing info
Click Submit
Final Step: You’re Done!
The state usually takes 7 to 30 days to approve your application.
Once approved:
- You can print your medical marijuana card
- Or save the PDF to your phone to use at Kentucky dispensaries
