The Process

To qualify for your Kentucky medical cannabis card through the Kentucky medical cannabis program you must be a Kentucky resident, Must NOT have any disqualifying felony convictions and have a qualifying condition which include…. 

• Chronic or debilitating pain (accident or injury post surgical pain, migraines etc)
• Any type of cancer regardless of stage or type
• Epilepsy or any other seizure disorder;
• Multiple sclerosis, muscle spasms, or spasticity;
• Chronic nausea or cyclical vomiting syndrome that has been proven resistant to other conventional medical treatments;
• Post-traumatic stress disorder 

1.  Fill out your intake paperwork we send to your email via our secure portal.  We will check them over to make sure you qualify.

2. Schedule and pay for your appointment.  You can pay through our website, we can text you link or we can take card by phone.  

3.  Get registered on the states website (YOU CAN START THIS BEFORE OR AFTER YOUR APPOINTMENT WITH US):  https://ky-chfs-public.nls.egov.com/register   (Your physician certification will automatically drop into your state account after your appointment with us)

5. Pay your $25 state fee through states site with a card or checking account.

After you see our cannabis doctor the state generally approves your Kentucky medical card within 7-30 days.  You can print it out from your account or save the pdf on your phone to shop at dispensaries.
